This information is being collected as part of the Central Consolidated School District's and JOM's continuing effort to understand your experiences with school, staff and services at your child's school.
Your responses on this questionnaire survey are voluntary and will be confidential. This survey will help us strengthen relationships between you, the staff and the services at your child's school. You can access this survey on the district social media and district website.
You will not be individually identified. There will be no follow-up contact from the school. Please think about your child(ren) and/or grandchildren who attend any CCSD school as you respond.
