Surveillance Testing
Dear staff,
 
As we move forward in welcoming our students back into the classrooms, the Central Consolidated School District is required to have 25% of employees randomly selected for surveillance tests each week.
 
Testing of school employees is to be provided using tests from the Vault home test kit (link you received) or Oral/Nasal Swab at McGee Park or a test at one of the Department of Health, Health Public Offices. COVID-19 testing is also available by Curative at McGee Park in Farmington, 7 days a week, from 10:00 a.m. to 3:00 p.m.  Please note, the San Juan County Public Health Office will no longer be offering COVID-19 testing.  These tests must be completed as soon as possible and results returned to HR and to the Principal at each school. This is required by NMPED to start At-School Instruction.

Some important items to know:
  • If chosen, surveillance testing cannot be declined by an employee. It is a condition of employment.
  • People that have tested COVID positive in the past 90 days are not eligible for the vault testing.
  • You can still report to work while awaiting the results, as long as you are not symptomatic.

  • Once you receive the kit you must complete the process as soon as possible.
  • If you do not have a laptop or camera available contact Marlena Harvey.
Vault Home Surveillance Testing directions: (If you choose this option)
 
Step 1:  Order your kit - Use your physical address if UPS will deliver there, if not use one of the school physical addresses below:
 
**Don't forget to bring your ID if you are picking up your test kit at one of the following locations.

Step 2:  Get your kit - your kit will be delivered to your physical address or the school address that you entered in your registration - If you used the school address, someone from the school will contact you to get your kit. 
 
Step 3:  Complete your kit - Collect your Saliva supervised, via zoom video call, per the instructions in the kit. 
 
Step 4: Shipping - Ship the kit back using the UPS shipping labels provided in the prepaid package. Please complete your kit the same day you receive it and drop it off at one of the UPS drop off locations or the school where you received your kit.
 
Step 5:  Get your results: You will receive your results 48-72 hours after your sample arrives at the lab.
 
Step 6: Email results: Please send your results to Marlena Harvey at [email protected] as soon as possible.
